  1. Develop a support system.

  4. Share feelings, needs and concerns.

REACH Portal Spotlight

Watch this short webinar to learn more about Noom!

Noom uses science and personalization to help you maintain a healthy weight by improving eating patterns and physical activity habits and fostering skills needed to navigate your daily life. We’ll help you better understand your relationship with food, how to be more mindful, and give you the knowledge and support you need for long-lasting change.

This program is open to all eligible employees and spouses covered by the Riverside Health plan. Noom is a designated Reasonable Alternative for the 2024 REACH Wellness Program, offering a Level 2 credit if you don’t meet or improve in 3 out of 5 key health measures.

To earn Level 2 credit, complete the following two requirements:

  1. Log at least two different activities each week. Activities include:
    1. Log meals
    2. Log weight
    3. Complete a day of lessons
    4. Message your coach, if you choose to have one
  2. Complete the above requirement for eight out of 12 weeks

The Fruit and Veggie Challenge begins April 1!

Backyard gardens, farmers markets and even the produce section of grocery stores are brimming with colorful produce. Fruits and vegetables of all kinds supply unique nutritional advantages that are beneficial to your health.

To complete the challenge and earn 25 Healthy Habit Points toward your Level 3 Incentive, you must record 80 servings of fruits and veggies before the challenge ends on April 28. You will have until May 5 to report for the previous week’s servings.

You can record your servings on your desktop or mobile device. Visit www.RiversideHealthcare.org/REACH to log in and begin recording your servings on April 1!

Login Instructions for First-Time Users:

Username: first name + last name (johnsmith)
Password: date of birth (mmddyyyy)

2024 REACH Program Design

The REACH portal not only offers the chance to improve your health, but also helps you earn your 2025 Incentive. Employees and spouses may participate in all three levels.


Level 1 – Due Aug. 31
Benefit eligible employee and insured spouse must complete the following:

  • 2024 Participant Consent Form
  • Online Health Risk Assessment
  • Upload Primary Care Well Visit Form with REACH lab measures
    • Note: REACH Labs must be completed at least 24 hours prior to your PCP wellness visit.

Health Insurance Premium Credit

  • $780 Employee
  • $520 Spouse*

*Employee must complete Level 1 for spouse to be eligible for spousal credit. All credits are applied to the employee health plan premium.

Employee Financial Incentive $100


Level 2 – Due Dec. 15
Benefit eligible employee and insured spouse must complete the following:

  • New in 2024, meet 3 out of 5 Key Health Targets
         OR
  • Make a 5% improvement from 2023 in Key Target
         OR
  • Complete an Alternative Standard for the missed Key Target

2024 Key Health Targets

  1. BP ≤ 130/85
  2. Triglycerides < 150
  3. HDL Cholesterol – Men > 40, Women > 50
  4. A1C < 5.7
  5. BMI ≤25 or Men’s Waist less than 40″ – Women less than 35″

Additional Employee Financial Incentive: $300

Additional Spouse Incentive: Raffle entry for a chance to win an Apple Watch
(Winner will be chosen in Dec. 2024)


Level 3 – Due Dec. 15
Benefit eligible employee and insured spouse must complete the following:

  • Self-report 120 Healthy Habit Points in 2024
  • Mammogram is required as part of these points for women 40 years and older

Additional Employee Financial Incentive: $120

Additional Spouse Incentive: Raffle entry for a chance to win a $50 gift card (Winner will be chosen in Dec. 2024)

Noom Weight is a weight loss program that helps you change your habits and mindset around food. Noom will be offered to all benefit-eligible employees and any Riverside-insured spouse. Participate in the Noom program and earn $300 as part of your Level 2 REACH Incentive and replace the previous WONDR program.

Coming Soon: Your 2024 REACH Wellness Portal

The 2024 REACH Wellness Portal launches Jan. 8! The REACH portal not only offers the chance to improve your health, but also helps you earn your 2025 Incentive. It is designed to support you and your family on the path to improved health and wellbeing.

In preparation for the 2024 program, the portal will be shut down starting Jan. 1 and opening back up for launch on Jan. 8. Beginning Jan. 8, 2024, employees and spouses can access the portal and begin completing activities to earn all three levels of incentives.
